Some people do the standard 3-finger one hand method with their thumb on down.
Hence, 4 fingers on one hand.

I play two hands, index fingers. But then again, I can't even handle the fast stuff like balloon fever yet. Just stuff like those dance ones - nova pulsar, dot death, and lovely lovely lovely - are definately easy with index. Maybe four finger is better for gradeus and balloon fever and those. Although lots of the streams aren't too hard with index on gradeus.

I play with two hands, seven fingers, one toe, and sometimes my nose if I'm feeling giddy.
But seriously, one hand, index, middle, and ring finger along with my thumb every now and then for those up+down combos is enough.
