1. Getting mugged (it's happened before, so...yeah).
2. Death of a close family/friend. I think I'm more scared of that than myself dying.
3. Being diagnosed with a terminal illness.

Every time I go lift up the toilet seat to take a shit, I have to check inside the bowl to see if there's any massive spiders that are gonna crawl all over my schlong as soon as I sit down

.........I had a very bad experience as a child in a public restroom...

On a more serious note, the amount I procrastinate legitimately terrifies me, which relates to my fear of not accomplishing something great during my life

I'm not particularly scared of death, unless my final moments are in some sort of excruciating pain (example: being torn apart by wild animals)
Other than that my biggest fears are needles and being alive when the world ends. The latter is near impossible unless nukes start flying, but needles are very real. I get the shivers just thinking about them. D:

Being violently confronted by someone/something I don't know/expect

3. Reaching the end of my life and realizing that I did nothing worth while.

oh god that reminds me of another fear I have

when I REALLY have to go, and I rush into the bathroom and the seat is up. I usually do two things at once:

1. Put the lid down
2. Sit on the lid

But I have this fear that something will go horribly wrong and I will squash my nads under the lid or something

1. The dark. ****ing Amnesia didn't help. It's the illusion that dangerous/terrifying things are hidden by it, rather than the darkness itself.
2. Being physically trapped. It's not claustrophobia, since I'm fine with sitting in a cupboard. If I can't move my limbs, I start to panic.
3. Death, I suppose.

I don't like mentioning strange fears like that though, because if people have OCD or something, it could be a contagious fear, haha!

Alien/Zombie/Undead invasions
I have dreams of them all the time and don't know what the hell to do, and then I wake up panicked because they eventually do shit to me. I'm not so afraid of it irl, but in my dreams it's always stupidly annoying when they pop up.

I don't know what else I am afraid of though.
