You have to be a Subscriber. Pay 3.50 and you can have your very own.
And You get a lot of extra things. It's Awesome!

No, not to get a custom one. If you are a music artist or stepfile artist you get something along the lines of "R1 Music Producer" or "R1 Stepfile artist" put in to your title.Comment

If you get 10,000 posts your custom title will change to "Certified Postwhore"
Play a certain number of games and you'll get some variation of "FFR Gamewhore" as your title.
The ability to set your own title is a perk of being a subscriber or a member of the staff.Comment
