Did you remember to refresh the game IMMEDIATELY after that Token screen popped up?
As far as I know, if you don't do that, you won't get it.
edit-- #1: According to your profile, you HAVE BB Revenge. The token is there.

*EDIT*Ok you got it, my theory doesn't matter anymore. I would like to know what determines song completion in ffr though. I still don't know how I got this token with out completing the song at first.
It would help solve things if you could post your score.

Song completion is considered as finishing the song by getting past the last arrow without your life bar getting to 0. Say there are 1000 arrows in a song.
If you hit 990 of them and you miss the last 10, you probably won't fail the song and you'll still "complete" it. If though, at the 990th arrow, your life meter is say, at 25%, then 10 misses might make you fail, thus you wouldn't have "completed" the song.
An easy way to determine if you finished the song is to look here in this topic: http://www.flashflashrevolution.com/...ad.php?t=34509. That topic will tell you how many arrows are in each song. Add your perfects, goods, averages, and misses together and if that number equals the number in the above topic, then that means you probably passed it.
