bzt, it's a parallax. Just because it looks like you're hitting the arrow perfectly, for what you know, it could be a small frame mess up and you'll hit it late enough for the avg-miss
The first part to understanding an avmiss is that you must realize that the second arrow takes precedence over the first arrow in the FFR engine. FFR runs on 30 frames per second, so one frame is 1/30s or .033s, which is rather wide for a music game.
Now, the judgement window goes as such: [a][g][p][p][p][g][weird stuff]. I'll ignore that last frame in the next part.
Now, an avmiss occurs when you hit an arrow inside its timing window but also in another arrow's timing window, you get an average for the second arrow and the first arrow gets completely ignored, thus giving you a miss. To visualize:
[a][g][p][p][p][g]
................[a][g][p][p][p][g]
This is what a 4-framer looks like. If you were to try to hit the first arrow on the red frame (the frame that the arrow actually appears onscreen), then the game will ignore the first arrow and give you the average for the second arrow, while the first arrow scrolls off the screen as a miss.
