I thought that's what was happening in some songs. At first I thought it was an unsync'ed double, then after seeing it a while, I realized it was intentional because it matched the music. It usually throws me off, but I'm not that great, so I'm not that surprised.
Regarding 1/192 notes, I can't ever remember seeing them in any music I've played, and I've been playing music since I was about 5 years old. I can't imagine them appearing unless they're used for musical grace notes (look it up on Wikipedia or see some of my remarks and responses to them in the Trills thread). Have people actually used 1/192 notes in step files? Is that why they exist and have a color?
