It did that to me once on Neomax (on the game level stats). I tried to get it to happen again, but I can't, and I don't know what causes it, but for me, it won't even happen again on Neomax.

just means the site is down. which is was. so it couldn't connect to the db to record your score. it'll work now.

Hmm, I got 2 Goods. Had a run with 1 good 1 miss, lol. The miss was like the 6th note before the end of the final stream of the song, so dumb. Oh well, 2 is good enough for now =)
