For anyone who mentioned the new Ocarina of Time: I hope you realize that all they really did in that game is change the graphics. They made no attempt to make it any different from the original. I understand it's a remake, but my God the most you could of done Nintendo is at least make us a new orchestral soundtrack for the game. The Nintendo 64 had a lot of restrictions so reusing the soundtrack from the original is just plain lazy, no offense. I'd rather mention Ocarina of Time (as in the N64) if I was to say it was the "best game of all time".
But on topic. I'd have to say the best video game ever made in gameplay and originality would have to be a tie between Tales of Symphonia and Chrono Trigger. If I had to go on a game that's less known though I'd say Lufia 2: Rise of the Sinistrals. The remake in this case on DS is fine.
edit: Holy fuh re-reading the thread I totally forgot about the DKC trilogy, I don't think I ever spent that much time playing a platformer in comparison to any of those.
NES: Final Fantasy
SNES: Donkey Kong Country 2
N64: Legend of Zelda: Majora's Mask (that's right, its not OoT)
Gamecube: Super Smash Bros. Melee
Wii: Super Mario Galaxy 2
Gameboy: Super Mario Land 2
GBC: Pokemon Gold
GBA: Megaman: Battle Network (series)
DS: Mario Kart DS
Dreamcast: Chu Chu Rocket
Playstation: Spyro trilogy
PS3: Metal Gear Solid 4
I'm rather against nominating games between multiple genres under a single title of "best video game ever made". The range is just too difficult and is pretty subjective to the player. It's like nominating a best book of all time, or a best movie of all time: there are a lot of great books and movies, but singling out one title as the "best of all time" is largely disputable imo
I feel like "what's your favorite video game" is a more acceptable question. Sorry for the off-topicness, it just irks me sometimes when I see this being done sometimes.
Aside, I grew up very much with the Gameboy Advance (well, emulator, but yeah xd); there were just a lot of really amazing games that I really enjoyed from that system.
Favorite(s) would probably go to Metroid Fusion, Castlevania: Aria of Sorrow, and Fire Emblem.
I think that if I had to say something about my favorite game now, it would most likely be Starcraft / Starcraft: Brood War. Fantastic storyline/campaign and really awesome multiplayer to boot. I was pretty shit at the game when I first started but I got loads better, still couldn't break D+ on iccup though lol xd (then again, D+ is a rather big achievement compared to the vast majority of SC players) The map editor pumped out really nice maps too that made the multiplayer even more enjoyable.

Why in the **** would you ever wanna change it @_@ I'm already ticked that they nerfed the water temple.
Also 3 games I forgot to mention are Zombies ate my Neighbors, Perfect Dark, and Goldeneye.
Jesus, I can't name just one. I can kind of break it down via a few categories though.
Final Fantasy 6 probably takes the RPG category for me. There are so many goodies, but this one stands out for me.
Link to the Past was the best Zelda IMO and is definitely up there. Hell, in terms of ranking Zeldas for me personally: LTTP > Majoras Mask > Twilight Princess > Zelda 1 > OOT > Wind Waker > Awakening > Adventure of Link > Minish Cap > Oracles (Haven't played the two DS ones yet!). I love OOT, but I don't think it's the best of the series (Keep in mind that Minish Cap and Oracles are still super fun despite being at the bottom; they're all good games).
Favorite side scroller is probably Mega Man X. I've beaten this game a million times. It's ****ing awesome, though I have to say Donkey Kong Country Returns is balls to the walls good too.
Adventure/collectable favorite definitely has to be Banjo Kazooie. Even the absolute awesomeness of say, Mario Galaxy 2 doesn't top this classic for me. Such a great game!
SNES - Megaman
N64 - Donkey Kong 64
PS1 - Tie between Crash Bash and Crash Team Racing
PS2 - Tie between Starwars Battlefront 1 & 2 and Ratchet and Clank series
GBA - Megaman Battlenetwork Series
DS - Tie between Mario Kart Ds and Megaman Starforce series (Starforce storyline sucked though)
Wii - Super Smash Bros. Brawl
Xbox360 - Tie between Battlefield Bad Company 2 and MW2
There are lots of other great games that I have but these are the ones that are standing out im my mind right now.
Best AAA = Aballava(?) + Serious Shit (97) Best SDG = Fushigi Usagi Milk Tei 8-0-0-3 (101) Best Score = do i smile? 3-0-0-3 (100)
