Actually, the difficulty in SM can go as high as you want. You just have to make the change manually in the .sm/.dwi.

Most of the regulars on these forums can easily beat Guru songs. It's all about Full Combo's and AAA's

I'd have to agree that FFR is all about full combo's and AAA's. I can comfortably beat 15's and 16's on SM (like Reach's Reality, or Zaghurim's Jai Envie de Plaisir X2)--but my FFR rank isn't that good because people constant AAA songs everyday, whereas I black flag or green flag songs.
