Well i was gonna say use a position thats comfortable for your arms/wrists because they will get tired just like any other position. But since it's spread, you have control over the way your arms are spread, compared to using the arrow keys where you dont have a choice since they're all bunched together. I use z x NUM2 and NUM3 so my arms are spread out to give me some comfort and relaxation, so that my arms aren't forced in one position that tenses my wrists after some playing time.
When my girlfriend first switched to spread (she was originally one hand 3 fingers) she complained about how her left hand couldn't do ldld or dldl combinations due to pain, I told her to play through it until it doesn't hurt.
That's the best tip I can give you, just work through the pain.

yeah my girlfriend plays SM but she can only do 6 footers atm, she plays index style but with her 2 forefingers.
I play spread, "q w num8 num9" I know what you mean about your left hand, I can't really offer any advice but you will get used to it, I did and I'm still improving I'm certainly not a pro though imo..

But concerning pain and whatnot, being left-handed obviously helped me haha, but do this. On any song that has urur or ruru combinations, set "mirror" option on in options and play them with your other hand, do this until it gets better, that does help, or do the opposite so it's off your weak hand.
