I started out playing with one hand, but I quickly switched to two hands because shortly after I switched I was able to AAA a lot better and get higher combos.
2 handed is the only way to efficiently play this game xD I mean by all means if you want to stand out and become a 1 handed pro have fun spending many upon many years achieveing horrible joint conditions
Two handed on the arrow keys. "Real" spread would be better since it prevents your fingers from being cramped together, but I have thin fingers so it's all good. Also I never have to do one-handed trills.

I usually play one handed, but my num pad does not like hands or quads. Hence, I forced myself to learn spread so that keyboard files would actually become playable. Sometimes I play index too, so technically I use either one hand or two hands in two different ways.
Not true. I know that you can't AAA a lot of C,VC,FMO songs if you are 1 handed, however, even being able to legitimately FC FMO songs with 1 hand is actually PRO. I used to use only 1 hand and believe it or not, I could FC FMO songs without mashing. (it's not easy)
You should use two, unless you restricted to using one. Ultimately it's whatever works the best to your liking. Spread is the most popular which would be two fingers on one half of keyboard & 2 on the other.
