Some of them are people who hacked the token, then hacked a score on it. They all got banned/wiped, to my knowledge

Super easy to be honest, and a trainer is probably the most outlandish way one could do it. All you need to know is very BASIC understanding of php and you can just go ahead and add shit right into the t-xmlfeedbacktest file (ergo, the song list).
As far as what Dossar said, it is definately possible to just play a completely different file and get an AAA that way. It would probably take all of 2 seconds to make a SM file with that many notes (or find a level that already has the same amount of notes), convert, replace when loading, and AAA.
PS: Playing a token song you don't have doesn't give you the token or anything. So anyone who hacked St. Scarhand while having the token for it also hacked the AAA on reality to obtain it.
