As you get better at more difficult songs, your overall PA will start to improve, and you'll be able to go back to easier songs and have a much less stressful time AAA'ing them. My advice would be to keep moving forward in terms of FC'ing harder stuff, then going back.

I don't use Vista, so there're no lag problems with the game. My keyboard also works fine. As far as I can tell, the problem is me.

I've always played two-handed and I already use speedmods, but I've never used screencutting. What is it exactly? I thought that was covering the lower half of the game, but I don't see how that could help.

I'm not bad by any means; as I said, I can fullcombo plenty of Very Difficult songs. It's just that, no matter how easy or how hard the song is, my perfect accuracy never goes higher than 90% or so. I can hit all the arrows, but not dead-on.
As was said before, if you can only FC Very Difficult songs, move onto Challenging songs. Look for the easiest Challenging song you can find and try to FC it. Keep moving up like this and eventually you'll be FCing FMO songs. Then, you can try to AAA easier songs.

What I did is that I kept trying to full combo harder and harder songs. When I was able to full combo most of the challenging songs, I went back and tried to AAA the extremely easy (and now boring) songs. It worked great, and now I can AAA a whole bunch of stuff. I'm just too lazy to go back and do easy songs lol.
