I have a routine of:
- Set speed mod
- Lower quality
- Run key remapper and turn it off (so I can still type in multiplayer)
Just before I'm starting a game, I always double check to see if the key remapper is on. It hasn't happened to me yet, but I'd hate to lose a game because I'm pressing buttons that technically don't send the arrow keys you want.
I don't have any physical warm-ups, though.Comment

-Change resolution to 800x600.
-Open up FFR the game on IE.
-Turn on FFR keymapping and AHK (which turns off the default keys).
-Turn on speed mod.
-Set up keyboard and monitor to exact desired position.
-Pound on the desk/side of keyboard for a moment.
-Start up a game and make sure the window is scrolled far enough to the right.
-Begin playing.Comment
